I know this post is from a while ago I just have a couple questions. Does your rash stay for a long time? Does it get really hot?
I get a really hot face, it burns, bug sometimes it only lasts a few hours. It does look very similar to yours. Very red, but like I said doesn’t last long. Just curious.

The malar rash can stay for a couple of hours or a couple of days, no real pattern to it. I have had the burning hot red face before, too. Mine only stays hot for a few hours as well. I found out that I have a condition called erythromelalgia that causes burning red skin on my face, chest, ears, hands and lower legs and feet. This is different from the malar rash, which only happens when my lupus is flaring, and doesn't feel hot. The doctors who know the most about erythromelalgia are usually dermatologists. It's like the opposite of Raynaud's syndrome (which I have also), and Raynaud's and erythromelalgia are both neurovascular conditions.
